At OT Links, we take a collaborative approach to our assessments, and aim to combine our clinical skills with your self-knowledge to produce a report that is both relevant and useful.

A FCA provides the NDIS with a baseline or an update of your current functioning to help inform your NDIS plan and funding.
Functional Capacity Assessment (FCA)
This assessment illustrates the impact of disability in accordance with NDIS eligibility criteria and provides recommendations for reasonable and necessary services and supports.
Evidence of Eligibility (NDIS)
A SIL provides a roster of care and support recommendations to inform funding for participants with higher support needs. This support package enables a participant to continue living as independently as possible in an assisted living setting.
Supported Independent Living (SIL)
An ILO assessment provides a personalised care plan for a participant who wishes to receive tailored support in a home of their choosing.
Independent Living Options (ILO)
We can provide an occupational therapy assessment tailored to a specific need or request (eg. A meal preparation assessment to evidence the need for a funded meal delivery service).

Occupational Therapy Assessment (Specific)
This may include requests for specific services or items (low cost assistive technology only) to be funded through a participant's NDIS plan, or advocacy to revise existing supports.

Based on your preferences and location, we can offer a range of options to accommodate your specific needs. These include: home visit, telehealth (your your choice of platform) or clinic visit.
About The Occupational Therapist
Rachael is a Senior Occupational Therapist registered with AHPRA. She is also an endorsed Better Access to Mental Health (BAMH) provider and NDIS clinician, specialising in mental health and disability.

Rachael has previously worked across community, inpatient, forensic and psychosocial rehabilitation settings. She has a special interest in supporting those with various forms of neurodiversity, including autism, intellectual disability and mental health conditions.
